All Covenant Light Co-Op Families are required to submit the online Family Agreement and Waiver of Liability each year. Please click each of the links below to read and submit each form for the 2025/2026 Co-Op year. After signing each form, please type your name below to acknowledge the completion of each form. Placing your name in the space provided serves as your digital signature.

By electronically signing this document, I, the undersigned, confirm that my signature applies to all members of my household, including minors and adults, participating in Covenant Light Co-op activities. I affirm that I have the authority to sign on behalf of myself, my minor children, and all participating household members.

I understand that my electronic signature is legally binding, and I agree to be bound by all terms, conditions, and policies outlined in this and any related documents I sign electronically for Covenant Light Co-op activites. This signature applies to all documents I sign, unless otherwise stated in writing. 

I waive any requirement for a physical signature or delivery. 















Payment Instructions

Enter the number of students you are enrolling, nursery age through 12th grade. Payment is due within 5 days of enrolling. Payment will secure your spot in co-op. This payment will be applied towards total tuition for the school year.
